2em;'>4.医疗机构的名称、地址、法定代表人或者主要负责人、所有制形式、诊疗科目、服务方式等实际设置与医疗机构执业许可证或诊所备案凭证记载事项不一致；</section><section style='line-height: 24px;text-indent: 2em;'>5.超范围诊疗，实际开展诊疗项目与《医疗机构执业许可证》核准登记项目或备案的诊疗科目不符；中医(专长)医师使用的技术方法和治疗病症超出其执业范围；</section><section style='line-height: 24px;text-indent: 2em;'>6.买卖、租借、转让《医疗机构执业许可证》，使用非卫生技术人员从事医疗服务；存在聘用受刑事处罚后未按规定重新培训、注册的医务人员情形等；</section><section style='line-height: 24px;text-indent: 2em;'>7.出租、出借《医师执业资格证》，医师超范围、违规多点执业活动等；</section><section 2em;'>2.未亲自诊查、调查，出具虚假医学证明文件以及与自己执业范围无关或者与执业类别不相符的医学证明文件；</section><section style='line-height: 24px;text-indent: 2em;'>3.以虚假诊断、夸大病情或疗效，欺骗、诱使、强迫患者接受诊疗；</section><section style='line-height: 24px;text-indent: 2em;'>4.过度检查、过度治疗和过度护理，大处方用药、重复检查、滥用治疗手段等；</section><section style='line-height: 24px;text-indent: 2em;'>5.违规用药或输液、抗菌药物使用不合理、违反肿瘤用药原则等；</section><section style='line-height:
